WTF is going on with formatting?

One of my reviewers PM-d me to say an older story of mine had been stripped of its paragraph breaks and sure enough it had: had to copy/paste and reupload the whole thing with all of the pg breaks reinstated. WTF? Has anyone noticed this?

It did happen a  while back in a WIP I was posting but it only seemed to occur in a couple of chapters and then stopped happening.

Holy crap, that problem's back from the grave?! I thought they got rid of that for good a year and a half ago!

Quick question, what were you using as breaks?

Was it (---), (___), or (***)? Or (other)?

Hey thanks for replying! It was *** and that's what I went in and reinstated too... hope they aren't going to disappear again, because it's 22 chapters... :-O

That's creepy...My story was the same in chapter count when breaks dissapeared the first time. Of course, I was using (---), but still, it's creepy. Maybe the site can't handle that specific chapter count in a fic, so every time it happens, someone has a problem with the site. Huh...That could explain quite a lot, actually.

Though I'm wondering, has (___) ever worked? I've seen a couple people have (___) looking lines, but I've never gotten it to work for me.
